Please print, read and sign the Registration Form and then fill out the Dog Profile Form. Bring both forms with you to the onsite interview. No appointment required for interview, walk-ins are welcome.


Requirements
Vaccinations: Proof of the following vaccinations will be needed for admittance
  • RABIES, DHPP (Distemper, Hepatitis, Parainfluenza, Parvovirus).This vaccination is required per your veterinarian's recommendation.
  • BORDETELLA – the most common cause of kennel cough. This vaccination is required every 6 months or annually depends on the method your veterinarian uses.


In order to keep your dog safe and disease free, we have a few simple rules that we like our canines to meet.

Dogs must be:

  • Three months of age or older
  • Have up-to-date shots and records from your vet
  • Spayed or neutered if ten months or older
  • Friendly, generally social and non-aggressive with people and other dogs
  • In good health, free of ticks and fleas and on a preventative medication
